Qualifications:

• Education: LSW required, MSW preferred
• Experience: One year experience as an LSW. Experience with Hospice and/or Discharge Planning preferred. Experience with therapeutic counseling also preferred.

Job Description:

• Would be shared between Case Management and Hospice departments.
• Would function in role of discharge planning in acute/skilled care setting, therapeutic counseling in outpatient setting, admission role in Hospice setting.
